NHS Foundation Trust
NHS Foundation Trust runs a Community Diabetes Service made up of a team of specialist nurses, dietitians, and diabetes educators/facilitators. This service supports people with Type 2 Diabetes to manage the condition through referrals and people can self-refer through the website for information sessions. Referrals to the community diabetes service are made through a GP or practice nurse. There is a community clinic in Cheltenham and home visits are possible for people who are housebound or in nursing homes. For Type 2 Diabetes there is a team at the NHS Foundation Trust where there are a number of clinics.
